# Audit Binder Index

Appendix C Kit 9 as a working, multi-page index. Use it when an AI-assisted task could be asked about later. When in doubt, it gets a binder entry.

## The Index

```
Date | Tool used | Purpose | Prompts used | Sources checked | Review sign-off
[...] | [Exact tool, account type, and tier] | [What the task was] | [The prompt, or a description of it] | [What you opened and verified] | [Name, date, one line on what you reviewed]
```

## The Rules

- The binder holds method, not data. If the task touched regulated data, the line records the account and applicable Chapter 3 checks; it does not store the data.
- Prompts that failed the Chapter 3 checklist do not get logged. They get redone.
- Nothing gets deleted in a refresh. Stale entries get an as-of date; the entry stays.
- The standard is one sentence: an auditor could reconstruct what you did without asking you a single question.
- Run the refresh on a fixed date each quarter: flag entries missing a sign-off or a named tier, then close with your sign-off, name, date, and what you reviewed. File the refresh note on top.
- The sign-off line is yours, every quarter.

## The Quarterly Refresh Note

```
Quarter: [YYYY-Q#]
Refreshed on: [Date]
Entries added: [Count]
Entries flagged: [Count and why]
Reviewed: [What you opened and verified]
Sign-off: [Name, date, one line on what you reviewed]
```
