# Kit 9: The Audit-Evidence Binder Index

**Use when:** an AI-assisted task could be asked about later. When in doubt, it gets a binder entry.

```
Date | Tool used | Purpose | Prompts used | Sources checked | Review sign-off
[...] | [Which tool and tier] | [What the task was] | [The prompt, or a description of it] | [What you opened and verified] | [Name, date, one line on what you reviewed]
```

The binder holds method, not data. If the task touched regulated data, the line records the tool and tier it ran in; it does not store the data. Prompts that failed the Chapter 3 checklist do not get logged. They get redone. The standard is one sentence: an auditor could reconstruct what you did without asking you a single question.

A multi-page working index, with the as-of rules for stale entries, lives in the kit's `templates/` folder as `audit-binder-index.md`.
