# Meeting Actions Tracker

Appendix C Kit 5 as a reusable tracker. Fill the table the day of the meeting. Monday's chase runs off it, so the truth of your week lives here.

## The Tracker

```
Action or decision | Owner (a name) | Deadline | Where tracked | Status
[One line per commitment] | [Name] | [Date, or none stated] | [Tool or file] | [Open / done / blocked / confirmed]
```

The three fields that matter: owner, deadline, home.

- No name, no action: an ownerless line goes on next week's agenda as a decision needed, not a task.
- If no deadline was spoken, write none stated and mean it.
- Status confirmed means done in the tracker, not done in someone's head.
- Never invent a deadline; if none was spoken, write none stated.

## The Monday Chase List

Run on Monday against last week's rows. From prompt card P26.

```
Item past its deadline or due this week | Owner | One-line status request
[Item, with the deadline]               | [Name] | [Ask for a date or a decision, one line]
```

Rules:

- List every item past its deadline or due this week, with its owner.
- Draft a one-line status request for each.
- Flag anything marked done that the tracker does not confirm.
- You review every line before anything sends.
